Camper Registration

We look forward to having your children join us for Camp Arevelk 2019!  Below you can find the 5 step process for registering your children for camp this year!  The registration forms, medical forms and payment need to be received prior to the start of camp.    

Step 1: Online Registration Form

Please fill out completely and submit to start registration process.

Step 2: Costs and Deadlines

Check to see what your cost will be based on when you register.

You can now pay your payment using our new online method. If you have any questions regarding this process or prefer to pay by check, please contact us at Camparevelk@gmail.com. The first early bird deadline is May 26, 2019.

Step 4: Fill out and print out Medical Forms.

*IMPORTANT REMINDER:* The New York State Board of Health requires that we have all necessary medical forms onsite PRIOR to the start of camp. Any camper whose medical forms are not received by the registrar by July 21, 2019 WILL NOT be able to participate in camp this year. THERE WILL BE NO EXCEPTIONS as camp can be shut down by the Board of Health if in violation of this requirement. Please schedule your doctor's visit and send forms to the registrar accordingly.

Step 5: Send forms to Registrar.

Send Medical forms, a copy of the most current immunization forms and a copy of the insurance card to Registrar. **NOTE that this address is in Canada - please make sure to allow extra time and extra postage so that your registration is received on time. **
